How can I create a city in another realm, for example 161 or 168 (which is not in the list at the entrance to accaunt)?

When a realm isn't visible to you on the realm selection page or in the footer of the game, then the only way to join it is if you have a registration link sent you by someone else. However, one realm you mentioned is an Italian realm and the other one is a Spanish realm. So I would suggest you to only join those realms if you actually speak the language of the realm. Next to this, both of the realms you mentioned are beta realms of v6. Currently their registration links are not working, so you can only join those realms if they are visible on the realm selection page or in the footer of the game.
