White Cross - Burned Fortress

Another noob question  but I promise I won't ask it again.... 

What does a white cross mean on a raised province?  Death?  Church? ..hmmm warrior monks?

Same topic... any other color crosses? Different meanings by color?

And there is only 1 color Cross. Just white, with some flames

Now I see a province with 2 crosses.  Twice pillaged?  I did not know that was possible. I thought there was a moritorium against hostility once someone was pillaged.

Can I attack a province that has a cross on it.  How about a province with two crosses on it?

I thought  maybe it was determent by the % of villagers killed. >10% 1 cross, <20% 2 crosses. Bu8t I could be wrong about this.
